Job Summary Provides clinical services at all Regional Alliance for Healthy Schools (RAHS) school-based health centers. Operates with considerable independence and works under general supervision of the RAHS Medical Directors, Advanced Practice Provider Manager and Director, and RAHS Director. Assumes responsibility and accountability for the primary care delivery within RAHS safety net settings for youth 21 and younger. Provide clinical leadership and expertise in the delivery of patient care, health education, and health promotion for the defined patient populations. An ideal candidate would be organized, enjoy working as part of an interdisciplinary team, have the ability to lead and motivate health center staff, and have a passion for working with an adolescent population. Responsibilities* Clinical Care: Provides skilled observations and health assessments of patients including history, physical examination, and appropriate interventions and referrals. Provides patient care services; appropriate to patient condition and in conjunction with current RAHS standards. Diagnoses and treats or appropriately refers health problems for the patient population. Performs diagnostic procedures as indicated. Establishes and facilitates plan of care related to identified problems, need and strengths including medication, diet, therapeutic procedures, counseling, education and teaching needs of patients, families, and groups. Assess and formulates plan of care related to preventative, acute, and chronic needs identified by the patient and health care team in a manner that provides continuity of care. This includes coordinating care with patients’ primary care physicians (PCP) when identified and identification of an available PCP when one is not currently designated. Provides case management services to integrate and simplify the range of services the patients/families receive within the program and community; identifies tangible resource needs and SDOH and facilitates referrals to community agencies. Works collaboratively with the health care team to provide complete and culturally sensitive health care. Collaboration with RAHS Clinical Nursing Director, Director and Co-Medical Directors: Work collaboratively to develop clinical guidelines, ensures utilization of current standards of practice, addresses practice issues, facilitates peer review and case discussion, biannual chart reviews, and QI documentation. Provides follow up on all deficiencies noted in reviews. Reports CQIP findings at All RAHS Program meetings. Program Development and Evaluation: Develop in collaboration with RAHS Director evaluation plan for RAHS including programs and services, ensure implementation and data gathering at all sites, participate in administering risk and needs assessments. Work with health center team members to monitor patient satisfaction surveys and develop plans for improvement as necessary. Compliance: Adheres to MM/TJC/MDHHS/CLIA standards. Maintains and remains current with educational models in M Learning, RAHS policies/procedure, and strives to meet or exceed MDHHS grant minimum program requirements, including quality benchmark thresholds. Participates in site reviews including Biannual pharmacy site visits, point of care rounds, TJC, MDHHS, or any other regulatory visit. University Students: Provides clinical educational experiences for graduate students and medical residents. Required Qualifications*
