Responsibilities*

The Cardiology Clinics is seeking a full-time LPN. This position will work closely with cardiology providers including Nurses, Advanced Practice Providers and Physicians to ensure timely and comprehensive care is given to cardiovascular patients. This position will focus on referral management, quality improvement monitoring, and post discharge follow-up.

Specific tasks for this position include but are not limited to:

• Review self-referrals to ensure that that patients are being scheduled with the appropriate clinic in appropriate timeframes based on clinical need
• Review General Cardiology new patient referrals to place appropriate patients with APPs to improve NP access
• Review Post discharge bridge appointment referrals that have not been scheduled or require clinical knowledge of subspecialties to determine the best place to be seen.
• Post-discharge phone calls
• Participate in Cardiovascular Medicine performance initiatives by review gaps in performance data ensure documentation supports action taken.
• Cross train to provide coverage for the other LPNs while someone is on vacation.

This job opening is for a permanent LPN position for the Cardiology Clinics.

Required Qualifications*

• Current Licensure as LPN in the State of Michigan
• 2 years of experience as an LPN.

Desired Qualifications*

• Experience with tele management of patients/ telephone triage
• 1 year experience with Adult Cardiology.
• 2 years experience in Ambulatory Care.
• Ability to problem solve
• Ability to prioritize tasks throughout the working day

NOTE: This position is directly responsible to the nurse manager and supervisor with functional reporting to the nursing staff in the post-transplant service.

Work Schedule

Hours per week: 40 hours

Shifts/Days/Hours: Monday through Friday 8:00- 5:00

Location: Cardiovascular Center, Ann Arbor, hybrid work available after orientation.
