Partners with local EMS and community health resources to conduct regular scheduled in-home visits to assigned clients for purposes of assisting clients in coordinating health services. Exams, screens, and treats under the essential functions of paramedic and RN licensure. Provides post-hospital release follow-up care including, but not limited to, monitoring medication, dressing changes, and checking vital signs
Maintains a consistent and timely documentation for delivery of services utilizing the medical record system designated by Mercy Health.
Supports case management development activities and participates in research related to healthcare issues, professional and clinical issues, medical costs and system problems/ issues. Collaborates with other programs and community agencies and serves as a consultant. Gathers information about patient’s home environment, family, relationships and social-economic conditions to assist to improve or restore their capacity for social functions. Makes appropriate referrals for all abuse, neglect, substance abuse, mental health and homeless patients. Makes follow up telephone calls to clients, referral sources, providers and/or community agencies when necessary.
Evaluates patient progress toward outcomes daily and revises the plan based off patient needs. Documents effectiveness of care/interventions. Promotes self-management to achieve individualized behavioral and treatment goals to optimize health outcomes. Maintains awareness of all diagnoses and seeks input when needed. Evaluates outcomes and initiates modifications in plans necessitated by the patient’s condition with physician approval, as appropriate.
Assessment: Demonstrates proficient assessment skills, including the physical, psychological, social, spiritual, educational, functional and pain/comfort level.
Completes initial assessment and reassessments within established time frames. Uses assessment data to prioritize patient needs. Completes patient assessment form and notifies appropriate parties of identified triggers. Notifies appropriate parties of pertinent patient information (i.e., allergies, weight, isolation, and oxygen use).
Provides clear, timely, concise and legible documentation of pertinent patient information. Provides reassessment documentation which includes changes in patient’s condition and response to treatment. Assesses nursing care administered by those under their direct supervision. Assesses patient for signs of abuse/neglect.
Within the scope of the position and RN licensure, answers patient questions and educates patients, directing work when appropriate – acting as a liaison between MHUH, Urbana Fire and EMS and community health resources.
Provides weekly patient status report to CP executive leadership team. Updates on patients enrolled in the program and any new patients that have entered in throughout the week.

2 Year/ Associates Degree
Specialty/Major: RN/BSN
Preferred Education:
4 year/ Bachelors Degree
Specialty/Major: Paramedic/EMT, Community Health
Licensure/Certification Required:
RN
Licensure/Certification Preferred:
EMT/Paramedic. Driver’s License

at least 2 years of ED experience

at least 1-year paramedic experience, RN case management experience, some knowledge of community paramedicine concept
