Responsibilities
As a Care Coordinator, you are responsible for coordinating all care planning throughout the continuum of care, directly working with clients, gathering, compiling and coordinating the development of the continuing care plan. Must be able to effectively communicate, collaborate and provide timely communication with families, referents and those of the multi-disciplinary team. Must be able to effectively prioritize work and be organized and efficient at time management, coordinating schedules, managing patient orientations and providing information and consultation as needed with internal staff, families and referents. Assist in providing all clients a smooth transition through the Hazelden Betty Ford continuum of care, effectively communicating to families, referent and clients about recovery management.
