The Medication History Technician (Emergency Department), working under the supervision of a pharmacist or nurse, is responsible for accurately collecting a current and complete patient medication list (includes prescribed, OTC, dietary and herbal supplements) for physicians to utilize during the admission process. The pharmacy technician is responsible for relaying any issue or concern identified to a pharmacist/nurse/physician. Comprehensive knowledge of medications is required with the ability to utilize drug information databases to obtain the necessary medication information. The pharmacy technician must possess excellent communication skills and problem solving abilities. The ability to handle multiple assignments , work independently and collaborate with the healthcare team are essential. Limited medication distribution responsibilities are required.
The hours for this position are 11am-9pm 4 days per week and every other weekend.
Responsibilities
A day in the life of a Medication History Technician (Emergency Department) at Hackensack Meridian Health includes:
1. Performs and documents medication history. Actively includes the patient and/or the patient’s family. Contacts outside pharmacies and/or prescribers to resolve discrepancies in order to obtain a full reconciliation in a timely manner. Utilizes multiple electronic databases to assist gathering medication history information.
2. Performs assigned responsibilities according to departmental policies, procedures, standards, and regulations within required time frames. Provides excellent customer service. Acts as liaison between the Emergency Department and the Pharmacy. Triages calls and questions effectively and efficiently to address issues and concerns where appropriate. Escalates to a pharmacist as necessary. Establishes and maintains positive relationships with patients, visitors, and other employees. Interacts professionally, courteously, and appropriately with patients, employees, and staff.
3. Ability to learn and utilize multiple computer applications to perform assigned functions and to fulfill job requirements. Documents medications and resolution of discrepancies in the electronic medical record. Adheres to appropriate procedures and notifies ISD and Pharmacy Management of hardware/software issues that are impacting workflow. Success completion of assigned hospital and departmental competencies and education by assigned due dates. Demonstrates knowledge of federal, state, and local regulations and standards (HIPAA, DEA, JC, et al)
4. Other duties and/or projects as assigned.
5. Adheres to HMH Organizational competencies and standards of behavior.
Qualifications
Education, Knowledge, Skills and Abilities Required:
1. High school diploma or equivalent
2. Two years of experience in community/retail pharmacy, hospital or medication reconciliation is required.
3. Must possess the following skills: drug knowledge, computer skills (basic PC/keyboarding), pharmacy calculations, medical and medication terminology
Licenses and Certifications Required:
1. PTCB Certified Pharmacy Technician.
