Job Summary and Qualifications
The Senior Manager of Complex Case Support is responsible for the daily management and support of the Corporate Case Management strategies for managing complex cases appropriate length of stay. The Senior Manager of Complex Case Support performs in-depth case reviews, collaborates with facility and division team members to overcome progression barriers and promotes a timely safe and appropriate discharge plan. In conjunction with the Director of Complex Case Support, the Senior Manager is responsible for collecting, analyzing, and evaluating complex case outcomes and operations data to facilitate the identification of gaps, support improvement opportunities, and reduce length of stay. This position requires an individual who is a self-starter and has the ability to manage multiple priorities, work with minimal supervision on projects and activities, demonstrates tact and diplomacy in situations of conflict and controversy and demonstrates excellent communication and presentation skills. This position requires the ability to function with minimal supervision in a rapidly changing environment
Major Responsibilities /Case Reviews
Performs in-depth case reviews on complex case management cases • Evaluates the current plan of care and the proposed discharge plan for patients experiencing extended length of stay by applying InterQual criteria, determining current / proposed level of care, assessing patient family resources, and evaluating interactions and decisions made by the multidisciplinary team. • Identifies and proposes strategies to address barriers to care progression and discharge planning, including financial, emotional, psychosocial, medical, and spiritual on reviewed cases. • Provides reports on findings, written summaries, and presents to leadership
Facility and Division Support
Provides direct support to facility case management team related to case management processes and workflows to meet established goals Acts as case management subject matter expert Provides ongoing case management education related to complex case management processes and workflow Assists the facility with action plan development as needed to address opportunities
Standard Compliance • Evaluates case management practices in relation to corporate, state, and regulatory standards • Maintains knowledge of corporate state regulations and industry standards
Education & Experience
Bachelors of Nursing required
Masters of Nursing preferred
3+ years of experience in acute care case management or relevant work experience Required • 6+ years work experience Required • Strong critical thinking skills with ability to identify root cause analysis and formulate solutions Required • Proficient data analytics experience; able to manipulate data for reporting and presentations Required • Knowledge and understanding of governmental, CMS, and JCAHO regulations Required Or equivalent combination of education and/or experience Licenses, Certifications, & Training: • Registered Nurse – Currently licensed as a registered professional nurse in the state in which he or she practices, in accordance with law and regulation. Required Knowledge, Skills, Abilities, Behaviors: • Complex Case Management: Knowledge of strategies and standards of care coordination; discharge planning; governmental, CMS and TJC regulations; post-acute requirements; community resources utilization; and hospital resources utilization (i.e. CFO, Physician Advisor, CMO, Legal, Ethics, Palliative Care) to manage complex cases and extended length of stay. Required • Analysis: Ability to perform data analysis and in-depth chart reviews/audits, identify barriers and root cause of length of stay issues
Required Travel expectation up to 25%
