About The Job
Care coordination is a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet the individual’s health needs through communication and available resources to promote cost-effective outcomes, using evidence-based guidelines as available in daily practice. The Care Coordinator I and II focuses on effective, safe, timely and complete transitions of care. In collaboration with the multi-disciplinary health care team, this position is responsible for the timely coordination of health care services to meet the patient’s individual needs and promote quality and cost effective outcomes.
This is a contract position.
