Responsible for organizing pharmaceutical care for the purpose of achieving definite outcomes that improve a patient’s quality of life.
Pharmaceutical Care
• The care that a given patient requires and receives which assures safe and rational medication usage.
• Inherently means that the pharmacist is responsible for and is to be held accountable for a patients’ achieving the desired clinical outcomes regarding the use of medications.
• It is ultimately the responsibility of the pharmacist to perform the functions necessary to attain the desired outcome of medication therapy.
• The goal is patient care outcome rather than the performance of tasks.
• Strategic Objectives Optimal therapeutic outcomes by ensuring safe and effective (rational) medication therapy
• Patient-focused care
• Quality procurement, preparation and distribution of pharmaceutical products.

EDUCATION, SPECIALIZED AND/OR TECHNICAL KNOWLEDGE REQUIREMENTS:
• Graduate of accredited college of Pharmacy with either a B.S. or Pharm-D.
• Licensed to practice in State of Ohio
• 2 years hospital pharmacy practice preferred, Experienced with Pharmacokinetics and Drug Utilization Evaluation (DUE). Familiarity with Unit Dose system, TPN and Chemotherapy.
