Responsibilities Home Health and Hospice cares for 400-500 patients in the community. Staff work in an interdisciplinary team that includes nurses, social workers, chaplains, home health aides, therapists (PT, OT, and ST), therapy aides, massage therapists, music therapists, bereavement counselors, physicians, nurse practitioners and volunteers to meet the needs of our patients. The department cares for individuals with healthcare needs in their home, which includes from birth to death and all types of medical conditions. The patient population is one of the most varied populations found in healthcare!The Home Health Aide/Hospice Aide is a trained person who provides personal care and assistance in activities of daily living for people in the place they call home. The Home Health Aide/Hospice Aide works independently under the delegation of a Registered Nurse and/or when appropriate a Physical, Occupational or Speech Therapist, and at times under their direct supervision. Qualifications License or certification: Certified Nursing Assistant, on State of Wisconsin Registry. Current BLS certification for healthcare providers.
