Under the general direction of the Clinical Leadership for Blake 11 and the Administrative Manager for Hospitalist Services in the Department of Psychiatry, the Resource Specialist will be embedded within the Blake 11 inpatient team. The Resource Specialist will play an essential role in the coordination of patient care, with an emphasis placed on efficient and exemplary service in a high-volume inpatient setting. The Resource Specialist is expected to effectively organize priorities, complete tasks proactively, and manage confidential patient information.
The Resource Specialist supports the social workers, physicians, and case management to ensure that the needs of the patients are met post-discharge. The Resource Specialist is responsible for scheduling outpatient care following the patients’ discharge plan, such as medication management, therapy and/or counseling. Oftentimes, this role also requires conducting research on CSS (Clinical Stabilization Services) and locating shelter for homeless patients and patients with a history of substance use. The candidate selected for this role should be flexible and must be comfortable with multitasking. The Resource Specialist finalizes outpatient care appointments and documents follow-up care details onto EPIC. This role requires communication, problem solving, attention to detail and an open mind.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
• Facilitate throughput
o Attend multidisciplinary rounds for assigned teams as needed
o Proactively check-in with social workers, physicians, and nurses as needed daily.
• Facilitate smooth, safe patient transitions from the inpatient setting to the post-acute or outpatient setting
o Work with multiple clinicians, including physicians-in-training, in order to facilitate appointment scheduling for patients.
o Troubleshoot within a complex system to resolve scheduling issues and track down physicians to make appointments.
• General duties
o Strategize with psychiatry inpatient teams and departmental leadership around process improvements for the inpatient psychiatric service.
o Leverage interpreter phone on a pole (IPOP) technology to communicate with patients and families who do not speak English.
o Maintain confidentiality and privacy consistent with HIPAA guidelines.
o Develop and maintain relationships with Operations Associates, Resource Nurses, members of the units
o Respond promptly to all calls and emails

EDUCATION:
• Associates degree required; bachelor’s degree preferred.

WORKING CONDITIONS:
• Inpatient hospital setting.
• This position requires the incumbent to stand and walk for extended periods of time as they move within and between several floors of the hospital.
