Massachusetts General Hospital is one of the leading academic surgical centers in the United States. As we are expanding our activities in robotic surgery across multiple specialties, we are inviting an experienced and motivated Physician Assistant to join our team for per diem coverage. The primary responsibility in this position will include surgical first-assistance in robotic surgical cases on services including but not limited to Urology, Gynecology, Colorectal, Thoracic, and General Surgery at MGH. This candidate will be involved in the training of residents, fellows and other PAs to perform the role of bedside surgeon in robotic cases, contribution to a robotic database including intra-, peri-, and postoperative outcomes, and assistance in clinical safety and quality improvement projects.
A competitive candidate will have at least 2 years of operating room experience, with preference for at least 1 year of dedicated robotic surgical experience or completion of an APPAP-accredited PA surgical residency program. Exceptional and motivated new graduates with rotation experience that reflects a dedication to surgery will also be considered.
Given the dynamic environment of an academic medical center with fluctuation in trainees and case volume across several sites, an ideal candidate will bring a high-degree of self-motivation, flexibility, and teamwork. We are a high-volume center, averaging 120 robotic surgeries per month.
PRINCIPAL DUTIES AND RESPONSIBILITIES
Primary management of mostly adult and some pediatric surgical patients in the operating room in conjunction with surgical faculty and staff, as well as senior surgical residents/fellows. Day-to-day duties will focus on directly assisting in multispecialty robotic surgery as the bedside surgeon with the operating surgeon and surgical trainee at the robotic surgical console. This role is in direct support of the operating room; there will be no floor, consult, or clinic responsibilities.
Operational:
• Preoperative
o Introduction and patient check-in, Q&A, site marking, consent review, history and physical review, knowing the pertinent patient details prior to surgery start.
• Perioperative
o General patient care, image review, preoperative huddling, monitoring devices, patient positioning, prepping and draping.
• Intraoperative
o Making incisions, robotic port placement and introduction, retraction, suction/irrigation, specimen handling, contributing to surgical decision-making, stapling and dividing major anatomic structures, suturing deep and superficial tissue layers.
• Postoperative
o Dressing placement, drain management, patient transport, postoperative order placement.
Administrative:
• Works with Lead Robotic PA and MGH OR leadership to help grow and develop the Robotic Surgery and Robotic PA program.
• Trains bedside personnel (residents, fellows, other PAs) in robotic procedures.
• Contributes to robotic database, including intra-, peri-, and postoperative outcomes.
• Assists in clinical safety and quality improvement projects.
• Works closely with OR Nursing to ensure best practices in perioperative patient care.
Educational:
• Participates in educational and quality assurance meetings for the Department of Surgery and Perioperative Services to identify areas of performance improvement in patient outcomes, patient experience, medical education.
• Assists with ongoing clinical research and quality initiatives as related to robotic surgery through follow-up of patients currently involved in clinical trials or via the robotic quality database.
• Assists in current clinical research and pursues independent clinical research involving robotic surgery and/or surgical outcomes.
• Advocates for and teaches surgical PA/medical students and junior residents rotating through the MGH operating rooms. QUALIFICATIONS
• Graduate of an ARC-PA accredited Physician Assistant training program. Bachelors and Masters degrees required.
• 2 years of surgical experience desirable (Thoracic, GU, GYN, Colorectal, General Surgery experienced preferred). 1 year of either dedicated robotic surgical experience or completion of an APPAP-accredited PA surgical residency program.
• Exceptional and motivated new graduates with rotation experience that reflects a dedication to surgery will also be considered.
• Must be certified by the National Commission on Certification of Physician Assistants prior to start date.
• Must be licensed by the State of Massachusetts prior to start date.
• Must maintain certification by completing required 100 hours of Continuing Medical Education credits every two years, to be sponsored by the institution. Must demonstrate continued competence by passing a re-certification exam every ten years.
SKILLS AND ABILITIES REQUIRED
• Qualified in all basic and advanced cardiac life support standards as established by the American Heart Association.
• Possesses the ability to follow complex patient management plans as directed by the attending surgeon.
• Eligible for state and federally controlled substances registration.
• Diagnose and treat health problems; obtain patient histories, perform comprehensive physical exams, do simple diagnostic tests, prepare a diagnosis and implement treatment procedures for illnesses; and prescribe medications.
• The staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his/her assigned unit.
• Excellent teamwork/people skills when dealing with patients and/or colleagues.
• Success in this role is contingent upon the presence of or immediate ability to acquire advanced surgical skills.
ORGANIZATIONAL RELATIONSHIPS
• This position reports directly to the Robotic Surgery APP Manager, Medical Director of Perioperative Services, and OR Nursing Director. PAs will be listed as being supervised by the surgical director of our PA program.
• Excels as a member of the surgical TEAM including attending surgeons, fellow/resident physicians, anesthesia staff, mid-level providers, nurses, students, and ancillary personnel.
• Works collaboratively with Nurse Manager and OR nursing staff.
SUPERVISORY RESPONSIBILITY
Supervises PA students or medical students rotating through the service. Teaches other health care professionals the management of robotic surgical patients.
WORKING CONDITIONS
The work may require the lifting of heavy objects and patients, as well as exposure to potentially infective material. Work requires contact with needles and other sharp instruments, electric-cautery devices, monitoring devices, and other state-of-the-art equipment. Prolonged standing can be expected. Walking to and from the patient care units can be expected. Hazards present from exposure to combustible anesthetic or disinfecting agents are minimized when safety precautions are strictly adhered.
