Serves as a member of the code call team by preparing medications and providing information; inspects and monitors medication orders and the dispensing of medications to the patient care units; prepares continuous narcotic infusions on the nursing units; evaluates and investigates orders for non-formulary drugs and makes recommendations regarding therapeutic alternatives; interviews patients to evaluate the severity and significance of adverse drug reactions (ADR) and allergies; documents ADRs, participates in medication reconciliationBachelor’s degree or Pharm D required
MA Pharmacy license required
