Medical Center: Rehab Core IP (Full Time, Day Shift) –

The Case Manager-Rehab is responsible for the accurate and timely completion of all Inpatient Rehabilitation Facility-Patient Assessment Instrument (IRF-PAI) and Centers for Medicare and Medicaid Services (CMS) Quality Measures paperwork for assigned teams as requested. Oversees the management and program development of assigned patient programs. Communicates with nursing regarding patient needs and acts as a liaison for the community physiatrist and consulting physician to ensure the patient’s healthcare needs are met. Maintains ongoing communication between team members, families and the insurance company to ensure ongoing authorization for services. Documents case management activities and utilization review as per requirements. Ensures regulatory standards are met for all regulatory agencies. Ensures completion of ongoing program evaluation, continuous quality improvement (CQI), program manuals, program development and documentation of these activities. These services may be provided for both the LLUMC, LLUCH, and ECH hospitals. Provides weekend and holiday coverage as specified by the Manager. Performs other duties as needed.
Associate’s Degree in Nursing or Bachelor’s Degree in Physical Therapy, Occupational Therapy, or Speech Therapy required. Bachelor’s Degree in Nursing preferred. Minimum two years of Rehabilitation experience and education or two years of rehabilitation nursing experience preferred. California License in Physical Therapy, Occupational Therapy, Speech Therapy or Nursing required. Basic Life Support (BLS) certification from the American Heart Association required.
Knowledgeable in coordinating and case managing the patient’s hospital stay from admission to discharge ensuring appropriate length of stay. Able to communicate positively with team members, physicians, referrals, case managers and colleagues. Ability to organize and analyze data. Able to acquire new data quickly. Able to read; write legibly; speak in English (and Spanish preferred) with professional quality; use computer and software programs necessary to the position; troubleshoot and calibrate patient care equipment. Able to relate and communicate positively, effectively, and professionally with others; be assertive and consistent in following and/or enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to communicate effectively in English in person, in writing, and on the telephone; think critically; work independently with minimal supervision; perform basic math functions; manage multiple assignments effectively; work well under pressure; problem solve; organize and prioritize workload; recall information with accuracy; pay close attention to detail. Able to distinguish colors and smells as necessary for patient care; hear sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with the work place/patient care; see adequately to read computer screens, medical records, and written documents necessary to position; discern temperature variances through touch.
