Humboldt Park Health Looking for Nurse Practitioner Care Van at Chicago Full Time

Position:  Nurse Practitioner Care -A- Van
Experienced Nurse Practitioner

The selected Nurse Practitioner will deliver a variety of healthcare services, including preventative, chronic and acute services.

  • Controls all aspects of scheduling site visits and client relations for location visited by the Care-A-Van.
  • Prepares administrative reports that include patient visits, volumes, trends, and referrals to Humboldt Park Health programs and services.
  • Oversees all equipment management including; medical supplies, office supplies, cleaning and maintenance supplies.
  • Assists with annual budget. Works with Foundation to provide information needed for grant writing.
  • Ensures van is properly maintained and licenses/plate/registration is up to date.
  • Provides medical, mental health, and health education services to patients and their families and care-takers.
  • Obtains detailed medical history and perform physical examination on pediatric patients.
  • Diagnoses and prescribes treatments both pharmacological and non-pharmacological.
  • Develops and implements patient management plans, records progress notes, and assists in provision of continuity of care.
  • Instructs and counsels patients regarding compliance with prescribed therapeutic regimens and health maintenance.
  • Maintains accurate records for clinical use and QA activities.
  • Collaborates with supervising physician and other members of the healthcare team, to include attending care coordination and other patient and clinical meetings.
  • Ensures lab testing and other testing equipment are working properly, including quality control.
  • Maintains vaccine management via VFC including attending mandatory meetings.
  • Proficient on vaccine administration and screenings (lead, hemoglobin, glucose, vision).
  • Ensure participation in continuing education and training to keep abreast of current developments in the industry.
  • Provide quality care to patients by following the most current evidence-based guidelines and procedure of practice.
  • Takes part in quality assurance audits.
