Care Coordinator I, Housing Specialist – TAY (Glendale Transitional Aged Youth)
This position is hybrid. The pay range for this position is between $24.24 – $26.93 hourly.

Summary
This position is responsible for facilitating stable living environments for Transitional Age Youth (TAY) clients of Our Third Place (OTP). This includes assessing clients’ housing needs, collaborating with various stakeholders and coordinating resources to secure appropriate and supportive housing options. This position will work closely with clients to develop personalized housing plans that address both short-term and long-term goals. Through collaboration with community partners and housing agencies, this position will create a network of resources that will help promote housing stability for OTP clients.
Primary Duties
- Completes client assessments and provides referrals for a housing match based on clients’ psychiatric condition, including substance use, social support network and readiness for independent living.
- Documents all services provided including maintains client records and program statistics as required by Didi Hirsch, various contracts, and other related agencies.
- Provides best match housing placement and assists client with interviewing for housing placement.
- Interfaces with the Department of Rehabilitation and other related community agencies as appropriate.
- Gathers/maintains available information and prepares routine reports related to housing placement activity such as client placement metrics, including successes and challenges to determine trends within the housing market.
- Consults with treatment team related to service coordination and service plans for clients referred for housing support.
- Monitors housing market activity to identify trends and prospective openings; keeps the program staff apprised of new housing opportunities.
- Ensures compliance with Didi Hirsch policies and procedures, state licensing, on other regulatory agencies.
- Develops and maintains housing placement skills through seminars, workshops, or other forms of training/education.
- Assists in identifying and participating in community outreach events and presentations for OTP.
- Attends and participates in routinely scheduled program, division, and Agency staff meetings.
Position Requirements
- Have a working knowledge of the housing market, AB34 programs, and placement of individuals that meet the AB34 criteria for placement services.
- Support the values and mission of Didi Hirsch as related to employment.
- Know and comply with Agency policies and procedures; HIPAA, Department of Mental Health, licensing policies and documentation guidelines; and other state/federal regulations.
- Demonstrate current knowledge of all job specific skills including working effectively with homeless or at risk of becoming homeless individuals with a history of mental illness; case management; psychiatric crisis intervention; documentation; multi-cultural and socio-economic issues.
- Possess demonstrated advocacy skills.
- Present ideas, information, and viewpoints clearly, both verbally and in writing.
- Efficiently use the personal computer word processing and spreadsheet software and client database program.
- Utilize analysis, experience, and judgment to make effective decisions within policies and procedures.
- Demonstrate commitment to team objectives and Didi Hirsch philosophies.
- Adapt to changing needs by acquiring new skills and knowledge.
- Current California driver’s license and a driving record acceptable to the Agency’s insurance carrier.
