Description
Mass General Brigham Healthcare at Home works with BWH and MGH in a collaborative partnership called Home Hospital. The program is based on our patients’ strong desire to maximize their time at home. When illness develops and is significant enough to require a hospital stay, Home Hospital is called upon to provide similar care for patients at home to avoid time in the hospital. You can think of Home Hospital as sending the hospital home with the patient. Home Hospital is more advanced than traditional home health care. Just like if a patient were in the hospital, this program would provide for several visits with our clinicians each day to treat the conditions until the patient is safe enough to be cared for by a regular clinical team.
How Home Hospital Works: Patients are assessed for Home Hospital by MD’s and Advanced Practice Providers (APPs). We have Program and Clinical Associates that assess the patient based on the geography and diagnosis, there name then gets sent to the Charge nurse and MD/APP who also assess the patient to make sure they are safe at home, have the correct diagnosis, meet inpatient criteria and the patient chooses Home Hospital care.
In this role the RN supports the Home Hospital Program based either at BWH or MGH and works in and up to a 15 mile radius surrounding the hospital treating patients in that area. (30 minutes out from Boston)
Job Summary
Full time, days 12 hour shifts
( part time and 10 hour shifts will also be considered)
evening diff from 5pm on
every third weekend rotation-weekend diff/all hours on weekend
2 major holidays
Provide quality, clinical services to patients in various settings in the community. Patient care is delivered within organizational philosophy, policy and standards of nursing and community health practice. The responsibilities are performed in accordance with MGB Home Care’s policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications. Skilled nursing care is provided in the form of assessment, teaching, treatment, documentation and care coordination demonstrating high quality customer service, and financial awareness.
