The mission of the Community Health Worker Initiatives
(CHWI) of the University of New Mexico Office for Community Health is to
contribute to the enhancement and integration of Community Health Workers
(CHWs) into the NM workforce in an effort to increase health equity. CHWI is
dedicated to designing, implementing, and evaluating projects that utilize
Community Health Workers (CHWs) as a strategy to increase New Mexican’s
well-being, promote health equity, and minimize the negative impacts associated
with the social determinants of health.
The following programs are looking to fill positions for Community
Health Worker in the following programs:
The COVID-19 Vaccine Health Equity Program is a state-wide collaborative program with the NMDOH and partner organizations that works to ensure that the COVID-19 Vaccine and accurate information is being shared with people who live in highly vulnerable communities that have been most affected by the COVID-19 pandemic. There are 3 positions available. These will be term positions through June 30, 2023 with an option to extend.
The Health Literacy Program is working to conduct education and outreach on COVID-19 and its vaccine in Albuquerque communities. Working closely with the COVID-19 Vaccine Health Equity Program, the Health Literacy Program focuses on working with CHWs in the community to ensure that high-level medical information on COVID and its vaccine are being shared with the community in a way that promotes understanding, trust, and confidence. The CHWs for this position will be located within UNM’s Office of African American Affairs and the City of Albuquerque’s Community Health Centers. Candidates with knowledge of the African American communities of Albuquerque are encouraged to apply. There are 3 positions available. These will be term positions through June 30, 2023 with an option to extend.
The Wellness-2 Family Shelter Program helps run operations for the City of Albuquerque’s Family Shelter. CHWs will support in both operational and case management needs in crisis intervention and resident support. Working closely with the shelter’s operations team and the on-site Acute Case Management team, this position will serve as an advocate and support to families experiencing homelessness. The ideal candidate for these 4 positions will have experience working with diverse populations, including individuals with Substance Use Disorders and Mental Health conditions. Using a Biopsychosocial framework, this candidate would work with shelter residents from a strength based, harm reduction and trauma informed perspective using a restorative and transformative lens. These will be term positions through September 30, 2022 with an option to extend.
The Intensive Case
Management Program (ICM) is a City of Albuquerque funded program within
Bernalillo County that assists individuals who currently have a substance abuse
issue with drugs and/or alcohol or who are at risk for relapse. Many enrolled high-needs clients are also
diagnosed with co-occurring disorders and have a significant history of
incarceration and Emergency Room visits. This role will connect these
high-needs clients to a wide variety of services around the community, provide
basic health education as appropriate, and meet people where they are at. This
role will perform street outreach for unhoused clients and work extensively
with community partner organizations to help clients access needed services and
resources. We encourage those with previous case management experience and
lived experience to apply. This will be a term position through June 30, 2023
with option to extend.
The CHW position within each program will interact daily in
the community to offer education and outreach for their specified program, as
well as offer resources and other community supports. Bilingual candidates are
encouraged to apply for all positions.

High school diploma or GED; at least 1 year of experience directly related to the duties and responsibilities specified.
Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.
