Serve a diverse and highly complex patient population as a key member of various interdisciplinary cardiology teams as part of the Pharmacy Department?s Cardiology Team; work to assist in the development, implementation, and monitoring of a variety of clinical services; ensure and promote the safe, rational, efficacious and cost-effective use of drug therapy in patients under the care of our cardiology teams; provide leadership in developing guidelines and policies, facilitating quality improvement processes, and ensuring the success of future generations through the clinical training of staff, residents and students.
Clinical pharmacy services are provided throughout Michigan Medicine to a diverse array of patients and include clinical pharmacy programs that complement our drug distribution system. Pharmacy Services have been organized to follow a service-line approach to better align pharmacy resources and provide the best health care outcomes for patients, which allows pharmacists to participate in teams that coincide with the way care is provided. Clinical pharmacy services offered and required of this position from a departmental perspective include code blue response, pharmacokinetic monitoring and dosing for specific drugs, order review and verification, anticoagulation therapy monitoring and patient education, renal dosing, parenteral nutrition monitoring, and comprehensive profile review. In addition, pharmacists provide leadership to ensure that the organization achieves core safety and quality performance metrics related to medication management.
Pharmacy Clinical Specialist – Cardiology
Pharmacist Specialists actively collaborate with the health care team in designing, implementing, and monitoring patient-specific pharmacotherapy on various cardiology services. The pharmacist attends daily rounds to ensure and promote the safe, rational, efficacious, and cost-effective use of drug therapy. This will include activities such as review of medication orders, provide dosage adjustments per pharmacy policies for anticoagulation, pharmacokinetic antimicrobials, renally dosed medications, parenteral nutrition, and conduct daily profile reviews. The development of guidelines and policies, quality improvement processes, research, and assistance in the clinical training of staff, residents and students is also expected. This position offers the opportunity to serve as a preceptor for University of Michigan College of Pharmacy Introductory Pharmacy Practice Experiences (IPPEs) and Advanced Pharmacy Practice Experiences (APPEs) and obtain adjunct faculty status within the College. Additionally, there are many opportunities to mentor and facilitate the growth and development of pharmacy residents through our strong post-graduate pharmacy education programs.
This will be a full-time position. The position will float through all cardiology services, primarily including: inpatient general cardiology, inpatient heart failure/transplant, inpatient medical ventricular assist device (VAD), cardiac intensive care unit (CICU), and outpatient heart transplant. Since this position is a float position, the pharmacist may be trained on other services (ie. transplant, CVICU) or in operational areas depending on departmental needs. The position is designed to provide weekend clinical coverage on a rotating basis. Weekend expectations are reviewed on an annual basis and subject to change based on departmental needs.

Responsibilities*
After successful orientation and training, the pharmacist specialist will primarily cover various cardiology services. The Pharmacy Department offers services 24 hours a day, 7 days a week, 365 days a year. This position will include primarily daytime hours, but may cover evenings on occasion.

Teaching
Pharmacists serve as preceptors for the University of Michigan College of Pharmacy students, for both Introductory Pharmacy Practice Experiences (IPPEs) and Advanced Pharmacy Practice Experiences (APPEs). Pharmacists are expected to mentor and precept pharmacy residents within the Department?s post-graduate educational programs.
Quality Improvement
Develop and implement quality improvement measures and process improvements to improve patient care and pharmacy services
