Summary of Job Duties:The Nurse Educator will work within the identified Family Medicine Clinic to promote screenings, early identification, and clinical navigation for the health and development of children prenatal through 5 years. Additionally, the Nurse Educator will be responsible for managing relationships with program participants, families and local community-based organizations. This person will track program progress and timeline deliverables to ensure the goals and objectives of the project are met, while also ensuring that relationships with participants and institutional stakeholders are maintained throughout the project. This position will engage in a range of activities such as outreach and participant recruitment, participant screening and assessment, health education, care coordination/service navigation, and community education and engagement.
Key Responsibilities:
55% Promotes Screenings, Early Identification, and Clinical Navigation for Children Prenatal-5:
*Directly supervises a Community Health Worker who provides clinical navigation, home visiting, and health education related to early childhood development under the supervision of Program Implementation Director.
*Work cooperatively as a member of the Family Medicine Clinic team to ensure project initiatives and goals are met.
*Partners with clinic team to coordinate and support overall health and wellness for pediatric clients and families.
*Ensures screenings, assessments, and treatment planning for pediatric clients are conducted according to chronological and developmental age.
*Coordinate referrals, support services, and/or treatment pathways for pediatric patients and their families.
*Submit timely and accurate documentation of services, billing data, and required paperwork.
*Support continuous quality improvement within clinics.
*Provide trainings and continuing education opportunities for clinic and community on early childhood healthy development.
*Participate in ongoing professional development.
*Attend all clinic and project meetings and trainings as required.
*Exhibits competency with technical skills and EHR documentation.
*Exhibits professionalism by maintaining a positive working relationship with patients, providers and employees.
30%Develops and manages relationships with program participants and community stakeholders.
*Coordinates communication with community stakeholders.
*Works with program managers to develop and deliver appropriate outreach and communication for project.
*Responsible for relationship building and management with external partners.
*Frequently tracks and reports on program deliverables through effective utilization of program management software.
*Identifies barriers and utilizes good problem-solving skills to address issues that arise in regard to participant relationships, ensuring all program objectives are met and data collection processes and procedures are followed.
10%Inform program and support clinic decisions as related to culturally adapting services to the Marshallese/Hispanic community.
5%Other duties as assignedQualifications:
Minimum Qualifications or Competencies (Essential for the job – the applicant must have at the time of hire) Basic competency include but are not limited to education, experience, licensed require, or physical requirements.
Education, general:
Associates Degree, ADN, or Diploma. Or BSN.
RN Degree Major: Registered Nurse.
Experience, length in years and kind:
5 years RN experience with 1 year experience in a clinical environment.
Licenses, certificates, or registration
Valid Arkansas Registered Nursing License. Current BLS certification or must successfully obtain certification within the first 30 days of employment.
Valid AR Driver’s License
Must be able to work evenings and weekends as needed
