The Population Health Coordinator will facilitate the planning, oversight and completion of projects. This role will develop and maintain project, program and communication of timelines, project plans, status reports and accomplishments. The coordinator collaborates with leaders and content experts to establish priorities, execute strategies and implement programs. This role requires the individual to incorporate continuous process improvement into daily work, using critical thinking and problem-solving skills to address opportunities for improvement while promoting engagement of stakeholders in solution development.
Principal Job Responsibilities:
· Utilize project planning tools and principles to define and manage project scope, monitor timelines and deliverables, along with communicating and identifying pathways to resolve risks and barriers.
· Proactively identify process improvement opportunities related to overall service delivery.
· Maintain multiple population health projects and initiatives to meet the quadruple aim of better care for individuals, better health for populations, smarter spending through improvement and care team well-being.
· Provide direct coverage for departmental personnel by performing the duties of a Care Manager or Patient Care Navigator when necessary.
· Represent Care Management on interdisciplinary committees and task forces as needed to promote the interdisciplinary team approach to patient care delivery.
· Promote successful outcomes with key performance metrics (KPIs) related to Care Management activities with enrollment activity and patient outcomes.
· Participate in education related to Care Management with physicians/providers, nursing, and other ancillary support staff to promote successful outcomes and coordination of care.
· Identify and resolve dependencies and constraints between and across projects by working with project leaders, key stakeholders, and management.
· Utilize business planning techniques to develop systems and processes that produce sustainable results.
· Effectively plan and facilitate meetings of varied participants using standard meeting management tools and techniques such as: agendas, meeting notes and issues tracking.
· Manage and facilitate complex decision-making processes with multiple stakeholders.
· Performs other duties as may be reasonably required from time to time.
Experience:
· Experience managing multiple priorities and healthcare related experience for a minimum of 5 years.
· Minimum of 2 years of current experience with process improvement and project coordination.
· Demonstrated experience managing multiple complex initiatives across organizational boundaries in close collaboration with leaders.
· Strong organizational, decision-making and problem-solving skills
· Excellent facilitation, conflict resolution and interpersonal skills.
Education:
· Bachelor’s degree or above is preferred or equivalent experience in healthcare for a minimum of 5 years.
