This is a non-paneled position to provide both virtual and in-person medical care.
Transitional Care Internist experienced and able to provide care across a continuum of transitions:
Responsibilities:
This position will require the physician to provide the pre-operative evaluation for medically complex patients requiring surgery.
The physician will assist the Bridge clinic in examining the following subset of patients: 1) POSH (Post-hospital/ED discharges), 2) Complex Care patients (medically and socially complex patients requiring intensive case management), and patients enrolled in the AMCAH (Advanced medical care at home) program, where virtual care is provided for qualifying in-patients to receive hospital-level care at home
The physician duties will also include working in the future Acute Ambulatory Treatment Center (AATC). This center will provide the monitoring, intervention, and treatment of patients with exacerbation of disease (such as COPD, CHF), those in need of procedures, and infusions. The position will involve mentoring APP and collaborating with a multidisciplinary team. There will be a potential for evening and weekend duties involved.
