Southern California Permanente Medical Group Looking for Post Hospital Discharge Clinic & Perioperative Care Clinic Physician at South Bay, California Full Time

Terms: 0.8 FTE

Qualifications: MD/DO; Board-Certified or Board-Eligible Internal Medicine physician with active California and DEA licenses

Experience: 

• At least (3) three years of inpatient post-residency training within the last 5 years AND / OR
• At least (1) one year of perioperative experiencewithin the last 5 years.

Job Summary: 

This is a non-paneled position to provide both virtual and in-person medical care.

Transitional Care Internist experienced and able to provide care across a continuum of transitions:

1. Pre-operative evaluation and assessment of patients planning to undergo surgery. The care will involve partnering and coordinating care with Anesthesia, Surgery and Medical subspecialties.
2. Post-hospital and post-ER follow-up care for high-risk patients. The work will encompass the Transitional Care space to ensure the coordination and management of these patients in the appropriate setting.
3. Virtual hospital at home (AMCAH) with hospital level of care provided within patient home, coordinated with the assistance of the Home Health program and case managers.
4. Acute Ambulatory Treatment Center (AATC) for care of medically complex patients requiring acute care in an ambulatory setting.

Responsibilities:

This position will require the physician to provide the pre-operative evaluation for medically complex patients requiring surgery.  

The physician will assist the Bridge clinic in examining the following subset of patients: 1) POSH (Post-hospital/ED discharges), 2) Complex Care patients (medically and socially complex patients requiring intensive case management), and patients enrolled in the AMCAH (Advanced medical care at home) program, where virtual care is provided for qualifying in-patients to receive hospital-level care at home

The physician duties will also include working in the future Acute Ambulatory Treatment Center (AATC).  This center will provide the monitoring, intervention, and treatment of patients with exacerbation of disease (such as COPD, CHF), those in need of procedures, and infusions. The position will involve mentoring APP and collaborating with a multidisciplinary team.  There will be a potential for evening and weekend duties involved.
