This position is accountable for professional social work assessments and casework, and providing direct services to senior citizens in the Montgomery County.
RESPONSIBILITIES AND DUTIES
1. Provides case management services to elderly clients.
a. Conducts client intake interviews.
b. Performs home and telephone assessments of clients for services.
c. Develops, maintains, and modifies service plans for individual clients.
d. Assists clients in establishing realistic goals.
e. Coordinates referrals to other agencies.
f. Consults and collaborates with medical providers and other community agencies.
2. Maintains records and prepares reports.
a. Organizes and records pertinent information relating to individual cases on a regular basis.
b. Furnishes daily activity logs.
c. Prepares monthly disposition summaries of the cases received, closed, and/or transferred.
3. Performs various activities relating to the development of the agency.
a. Participates in staff meetings, conferences, workshops, and training sessions.
4. Performs other related duties as required for the effective operations of the department and the agency.
REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
KNOWLEDGE OF: Social work practices, procedures and terminology; case management procedures; counseling techniques; Gerontology; community resources for older adults.
ABILITY TO: Develop and maintain effective working relationships with client, staff, social service agencies and the general public; organize and maintain accurate records and reports; develop and maintain client service plans; communicate effectively in both oral and written forms. Utilize a computer effectively for daily tasks and case management. Perform basic internet searches.
SKILL IN: Interpersonal communication. Microsoft Office applications.
QUALIFICATIONS
BA/BS Degree in Social Work or related field. LSW is encouraged. Prefer at least one (1) year professional social work experience.
Must pass a criminal background check and maintain a clean record.
