Your day.
As a Licensed Clinical Social Worker, you need to know how to:
Conduct psychosocial assessment interviews with patients to approve or deny readiness for bariatric surgery
Provide triage, assessment, and counseling for patients as identified as needing behavioral health intervention
Develop and implement treatment plans for stabilization and behavioral modification, mobilization and disposition appropriate for patient care.
Function as part of a multidisciplinary team to develop patient care plans
Provide services using accepted psychotherapeutic models and interventions
Provide consultation to primary care providers as needed and collaborate with staff and providers on treatment plans.
Consult and act as liaison with all other members of the treatment team to mobilize patient
Provide educational information to patients and their families
Provide aftercare planning
Participate in patient-centered departmental and inter-disciplinary meetings as well as non-departmental meetings and ongoing hospital committees as requested.
Maintain clinical skills through continuing education.
Complete all required competencies and maintain all health standards as required by hospital in a timely manner

Master’s Degree in Social Work
Licensed Clinical Social Worker in the State of Colorado (LCSW)
Knowledge and experience with managed care
Preferred:
Spanish proficiency
Previous experience in integrated healthcare, bariatric or primary care
Experience working with and sensitivity toward special populations, including those with limited income, limited education, and non-English speaking individuals
Experience with PC software applications in word processing, Epic, Google applications
