San Francisco Health Plan (SFHP) is a Medi-Cal Managed Care Plan serving the City and County of San Francisco. We were designed for and by the residents it serves – many of whom would not otherwise be able to obtain healthcare. Reporting to the Manager, Population Health Management, the Program Manager, Population Health will address issues of health equity in San Francisco through a focus on Cultural & Linguistic Services (CLS). You will accomplish this by developing, planning, implementing, and evaluating medium and large-sized projects and programs supporting SFHP, its members, and its provider network. The projects and programs help ensure SFHP provides a consistently high level of culturally and linguistically competent care, health education, preventative care, and chronic condition management with a constant eye to achieving health equity. Other areas of focus may be added as business needs arise.
What You Will Do
- Provide program/project management and leadership of multiple programs and projects simultaneously including:
- Develop, plan, implement, maintain, and evaluate programs and projects.
- Manage and analyze quantitative and qualitative data to support community assessment and member health outcomes.
- Regularly communicate to stakeholders, including facilitating meetings and making presentations.
- Suggest interventions based on data findings.
- Write reports, incorporating data.
- Deliver health education and trainings for diverse audiences.
- Make independent decisions on appropriate decisions involving manager and other stakeholders.
- Produce, monitor, and update all program/project documentation, which includes work plans, charters, budgets, meeting materials, policies/procedures, risk/decision logs, and presentations.
- Ensure that programs, projects, and their documentation follow regulations.
- Follow SFHP’s Project Management standards.
- Effectively collaborate with team members and a wide variety of stakeholders, such as peers, managers, directors, executives, members, regulators, providers, staff at other health plans, and community partners.
- Assign and oversee work of Associate Program Manager on select projects.
- Improve functioning of programs and projects by identifying opportunities, integrating work with other efforts, suggesting process improvements, and proposing solutions.
- Serve on Grievance committee and evaluate grievances for CLS content.
- Oversee Member Incentives program: implementing and refining processes, solving problems collaboratively, and communicating with other departments throughout SFHP.
- Work with vendors, including contracting.
- Plan, conduct, evaluate, and report on surveys and focus groups to assess cultural, ethnic, racial, and linguistic needs of members and SFHP’s efficacy in meeting them, including interpreter services and practitioner availability and concordance. Analyze data to recommend intervention strategies based on findings.
- Participate in audit activities of SFHP’s provider network regarding CLS content. Audits include reviewing policies and procedures, instructing others on regulatory requirements, and developing a plan in coordination with the provider group to correct any problems found during the audit process.
- Collaborate to create and maintain cultural awareness trainings for providers and staff.
- Serve as CLS Subject Matter Expert for additional program/project areas.
- Provide engagement and accountability for other CLS and health equity activities.
What You Will Bring
- Bachelor’s Degree in Public Health, Health Administration, or other relevant field, or equivalent work experience; MPH or Master’s in relevant field preferred or equivalent work experience.
- 3 years of working as a Program or Project Manager in a healthcare environment or equivalent work experience in similar area to achieve health equity.
- 1 year experience working on content related to Cultural & Linguistic Services or health equity; experience providing Cultural & Linguistic Services and thorough knowledge of CLS concepts.
- Experience with diversity, equity, inclusion, and belonging programs and principles.
- Expertise related to encouraging behaviors that promote health in a cross-cultural manner, such as developing clinical and operational interventions and possessing an understanding of health literacy and cross-cultural patient/provider communication strategies.
- Experience with data collection, analysis, visualization, and management.
- Knowledge of Medi-Cal, Department of Health Care Services, and/or CalAIM, or experience with Community Based Organizations is a plus.
