Overview An Advanced Practiced Nurse (preferably hold national certification in area of applicable expertise) with strong clinical and critical thinking skills, as well as excellent communication skills. The Transitional Care Coordinator will be knowledgeable in chronic disease management. He or she must be a true team player capable of and willing to support each team member in overall goal achievement of customer satisfaction and positive resident / patient outcomes. Goals: The Nurse Practitioner will be a driving force to ensure patient transitions are as smooth and as safe a process as possible, thus driving positive clinical outcomes, patient, family and physician satisfaction and preventing avoidable readmissions to hospitals. Patients identified, as high risk for hospital readmission will be a priority focus. Full time position at Southern Ocean Medical Center. (4) 10 hour DAYS, 7am-5pm, 2 weekends per month required. Work with primary care physicians, observation unit, rounding in hospital, all inpatient. Responsibilities Admissions: Evaluation of each new admission for diagnosis, medications, and medical plan of care to ensure completeness, appropriateness and to clarify any issues. Business Relationships: Develop ongoing positive, collaborative relationships with key customers: physicians, hospital leaders, care managers, social workers, home care etc. Care Plan: High risk patient’s clinical progress will be tracked daily to ensure care planning and physician’s orders are implemented. Clinical & Regulatory Practice Requirements: Clinical practice accurately reflects NJ Board of Nursing regulations, ANA Code of Ethics, the current NJ Department of Health requirements, and institutional policies and procedures. Practices within all NJ Board of Nursing regulations and procedures Establish effect communication directly with Physician on site and at least biannually with the Medical Director. Clinical Competence: Performs comprehensive initial assessment, history, physical exam and ongoing assessments. Consistently able to demonstrate clinical competence in the assessment of work related injury patients, health screening for employees with a variety of occupational demands, immunization services, including hepatitis B, Tuberculosis screening, etc. Maintains licensure with the New Jersey Board on Nursing as an Advanced Practice. Prepares all documentation requirements to remain in good standing for licensure and regulations for medical staff membership. Continuing Education: Independently participates in continuing education and maintains documentation. Develops a personal plan of development in area of clinical expertise, conferring with Medical Director on goals and objectivesActively seeks out opportunities for self-development on an annual basis. Seeks approval in a timely manner to attend programs to achieve educational goals. Documentation: Provides accurate, confidential, systematic and organized documentation of findings, recommendations/plan and education. Medical records are completed in a timely fashion with key elements of documentation consistent with sound Oncology Medicine principles. Documents all medical evaluations, diagnosis, procedures, treatment outcomes, education, referrals and consults consistent with the Joint Commission on Accreditation of Health Organization and state regulatory standards. Contributes to nursing communication handoffs and documentation standards: 24 hour report, initial admission, weekly and monthly summaries, progress notes, care plan updates etc. Provides job related reports and audits as requested by Director of Nursing. Plan of Care Development & Implementation: Develops and implements appropriate plan of care. Demonstrate wellness philosophy in treatment plan for oncology patients, Utilizes diagnostic testing, treatment plans and therapy to promote effective care to return to essential life-style. Patient Education: Provides appropriate education to clients, families, support systems and the general public. Provides medication safely and effectively to promote effective recovery from oncology care. Includes clinical trial information when appropriate. Facilitates patient teaching and support programs for patients/ and families. Effectively engages patient in wellness, survivorship, palliative or hospice care when appropriate Management of Disease Programs: Manages chronic disease programs and related projects as requested by Director of Nursing (eg. CHF program & COPD Program). Prescribing: Prescribes appropriate medication according to NJ regulation and scope of practice. Prescribes medication within all policy and regulatory guidelines. 45:11-49. Authority of nurse practitioner/clinical nurse specialist to perform tasks, order medications or devices, or make prescriptions; standards of joint protocols Prescribes medications and/or recommends over-the-counter medications as appropriate. Provides medication safely and effectively to promote effective recovery. Process Reviews: Attends Readmission Process Review Meetings, providing critical analysis to help team members identify issues and improve processes. Quality Assurance: Participates in QA and Oncology Health. Assist in data collection and analysis to promote effective practice for comprehensive oncology care. Quality Care: Ensures quality of care and customer satisfaction and will communicate any team member issues that compromise patient care and satisfaction to the Director of the Nursing promptly. Regulatory: Follows standard of care and clinical guidelines by keeping current with nursing literature and publications, seminar attendance, professional organization membership, networking and with federal and state regulatory guidelines. Has a basic working knowledge of State and Federal regulation requirements as they relate to LTC and subacute resident care. Policy and Procedures will be followed and upheld. Rounds: Rounds with attending physicians whenever feasible on all high-risk patients. Attends high risk patient and family care conferences as needed. Utilization Committee: Member of the Utilization Committee and will assist the social worker with high risk patient’s discharge planning as it relates to the continuum of care. rovides medical assessments, treatments, clearances for HMH applicants, team members, constituents and external customers per specific company profiles & or regulatory standards. 2. Performs clinical care professionally, serving as a customer advocate and role model. 3. Develops and implements appropriate plan of care. 5. Facilitates patient teaching and support programs for patients/ and employers 6. Evaluates current protocols and standards of care for translation of ¿best practice¿ into clinical practice. Supports the development of evidenced-based practice protocols and standards of care that include interventions and treatments to achieve desired outcomes. 7. Applies evidence and value-based practices to achieve positive outcomes for patients. 8. Provides medication safely and effectively to promote effective recovery from on-the-job injury/illness 9. Prescribes medications and/or recommends over-the-counter medications as appropriate. 10. Initiates laboratory tests, immunizations, and diagnostic test/procedures as appropriate according to regulation and scope of practice. 11. Demonstrates competent clinical skills with treatment plans using most effective screening test or procedure. 12. Collaborates with physician and consults effectively with other members of the health care team 13. Demonstrate effective communication skills interdisciplinary to achieve results 14. Provides accurate, systematic and organized documentation of findings, recommendations/plan and education 15. Independently participates in continuing education and maintains documentation. Develops a personal plan of development in area of clinical expertise, conferring with leader on goals and objectives. 16. Provides culturally diverse and educationally appropriate education in relation to patient care/needs. 17. Serves as a clinical resource to nursing staff, patients, and other multidisciplinary team members across the continuum to evaluate complex patients and identify appropriate interventions and evaluation of outcomes. 18. Participates in policy/procedure reviews for updating with current evidence-based and value-based practices 19. Acts as a resource to staff for addressing clinical, process, and system opportunities for improvement which promote excellence in care and clinical outcomes. 20. Promotes a collaborative work environment which supports the organizational mission, behavior expectations and values. 21. Promotes innovation within clinical practice by seeking opportunities to bring new clinical knowledge into practice. 22. Identifies ethical care issues and coordinates interdisciplinary collaborative meetings to resolve those issues. 23. Floats to various clinic locations as needed. 24. Other duties and/or projects as assigned. 25. Adheres to HMH Organizational competencies and standards of behavior. Qualifications Education, Knowledge, Skills and Abilities Required: 1. Graduate of an NLN/AACN accredited program in nursing 2. Master¿s Degree 3. Maintains clinical competencies through participating in continuing educational programs (contact hours per years), and clinical practice as mandated by Board of Nursing and national certification body. 4. Experience in relevant patient care setting or as regulations dictate. Education, Knowledge, Skills and Abilities Preferred: 1. Master¿s Prepared Advanced Practice Nurse 2. Minimum of 3 years professional experience. 3. Strong organizational and computer skills 4. Effective verbal and written communication skills 5. Emotional intelligence, critical thinking and effective interpersonal skills 6. Ability to adapt to multiple and changing priorities 7. Valid Driver¿s license Licenses and Certifications Required: 1. NJ State Professional Registered Nurse License. 2. Advanced Practice Nurse Cetification (w/ Master’s Degree). 3. AHA Basic Health Care Life Support HCP Certification.
