Job Summary
The RN Case Manager is responsible to the Clinical Manager and coordinates care for an assigned caseload of patients/families. The RN Case Manager is committed to the philosophy, goals, and objectives of Mercy Home Care (MNLH) and Mercy.
Qualifications – Minimum
Education:
Graduate from an approved school of Registered Nursing.
Current registration in the State of Ohio.
Minimum of two (2) years’ experience in Medical-Surgical, Oncology, Hospice or community health nursing is required.
A Bachelor’s Degree in Nursing preferred.
Maintains annual CPR certification.
Excellent oral and written communication skills.
Strong organizational and leadership skills.
Must possess self-direction, motivation, and time management skills.
Must be able to develop and maintain effective working relationships with people at varying ages, education, social, economic, and ethnic backgrounds.
Basic computer skills – word processing and spreadsheet applications.
Flexibility to adapt and respond to a rapidly changing environment.
Hours
Hours per pay period: 80
Hours: 8:00am to 4:30pm
Vacation rotation, weekend call rotation.
Primary role is Care Manager in office but some home visits as needed.
Home Care experience preferred.
