Responsibilities Essential Functions: Assembles charts: admission, transfer & discharge. Uses Electronic Health Record (EHR) to enter data for admissions, transfers & discharges. Communicates patient information to appropriate team members: nursing, other departments, providers. Completes appropriate forms: computer,paper documentation forms. Obtains records, charts, lab and x-ray reports via computer/ or paper chart. Performs receptionist functions with patient’s relatives and staff. Assists nursing staff as delegated in performing patient care activities according to established policies/procedures. Measures and records vital signs including blood pressure, pulse, respirations and temperature. Assists with Activities of Daily Living (ADL) including mobility, elimination, and nutrition. Assists in the proper lifting of patients utilizing Gait belt and/or E-Z lift or appropriate lift system as indicated. Documents patient cares in accordance with documentation guidelines (i.e., intake, output, height, weight, vital signs, flowsheets, comfort levels). Participates in departmental activities and pursues ongoing education programs to maintain and develop skills. Provides care appropriate to the age and ability of patients served. Maintains a clean and safe environment including equipment maintenance and setup. Helps in maintaining and organizing supplies and equipment. Practices Relationship Based Care Skills. Demonstrates knowledge of unit care standards. Completes all organizational/departmental requirements and education. #ELJ Qualifications Additional Qualifications : Successful completion of a health unit coordinator program or comparable experience (preferred). Current CHUC certification (preferred). Certified Nursing Assistant on State of Wisconsin Nurse Aide Registry in “non-expired” status (preferred). License or Certification : Basic Cardiac Life Support for healthcare providers- BCLS – certificate of course completion required and must not expire within 3 months after start date. A scanned copy of the certificate of completion or transcripts, including your full name, must be attached to your profile to be considered.
