Through the Intensive Clinical Program, brain health programs and regional outpatient clinics, Home Base serves the nation as a successful private-public partnership and as a source of communication and educational resource to health and community providers seeking to support our veterans. In addition, the Home Base program serves as a leader in research, identifying and implementing new treatments for post-traumatic stress disorder (PTSD) and traumatic brain injury (TBI) and other deployment-related mental health challenges.
Home Base is seeking a dynamic and entrepreneurial warrior to identify, motivate and guide into care veterans and families struggling with the invisible wounds of war. Reporting to the Director of Veteran Outreach, the Veteran Outreach Manager role is to manage a team of Veteran Outreach Coordinators and Lead Veteran Outreach Coordinators in providing peer support and outreach to veterans and their families struggling with the invisible wounds, with the goal of connecting them to care at Home Base. The Veteran Outreach Manager works closely with the clinical staff around case management of patients and the monitoring of patient needs while participating in the clinical and wellness programs. The Veteran Outreach Manager provides active outreach to the military community, working with Home Base Leadership to strategically identify greatest areas of need. The Home Base Veteran Outreach Manager will be part of a vital team of professionals and will serve as a critical “boots on the ground” liaison between The Home Base Program and the Veteran’s community and as an important leader, role model and mentor for the Outreach Team.
