Under the supervision of the Division Chief, Director of Nursing and Collaborating Physician, the Advanced Practice Provider (APP) will work in the Neuromuscular Neurology Division.
The clinical areas of focus will be within the neuromuscular division with a focus on myasthenia gravis and neuropathy, small fiber polyneuropathy clinics, immunomodulatory teaching and overall patient education. There may be additional areas of clinical focus in the future. The NP will perform clinic visits (in person and televisits), procedures (skin biopsies and lumbar punctures), and working closely with other departments and divisions leading clinical continuity. There may be additional procedures in the future to help support patient care. The APP will take a leading role in managing complex patients referred for assessment and care. The APP will help plan, refine, and extend care activities beyond the walls of the clinic to more fully integrate care and improve patients’ experience.
The position requires superb organizational skills, interpersonal skills, initiative and innovation. The ideal candidate is flexible, independent, and willing to participate in multiple activities while providing support to patients by working with the team. Candidates must have strong communication skills and the ability to converse comfortably with patients and their caregivers.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Clinical Responsibilities:
• The APP will provide patient care in eight 4 hour clinic sessions per week (32 clinic hours) and have two 4 hour sessions dedicated to administrative time (coordination of care, labs, documentation, etc).
• Collaborates with the provider(s) to provide targeted healthcare services to patients within the neuromuscular clinic population.
• Performs physical examinations and preventive health measures within prescribed guidelines and instructions.
• Develops and implements disease-specific patient management plans, records progress notes, provides home-going instructions, and assists in provision of continuity of care.
• Assesses, instructs, and counsels patients regarding adherence with treatments.
• Orders, interprets, and evaluates diagnostic tests to identify and assess patient’s clinical problems and health care needs.
• Monitors patients on immunosuppressive and immunomodulatory therapies within established clinic and national guidelines
• Provides care to identify and prevent side effects of medications
• Records physical findings, and formulates plan and prognosis, based on patient’s condition.
• Prescribes or recommends medications or other forms of treatment.
• Maintains accurate and timely notes in the electronic medical record.
• Manages communications with patients and responds messages in the EPIC in-basket
• Assists physician with comprehensive care and patient education.
• Self-directed and purposeful in seeking necessary knowledge and skills to develop and maintain competency within neuromuscular medicine.
• Performs other duties as assigned.
Facilitating Continuity of Care: Oversees and collaborates as necessary in the care of clinic patients.
• Provides clinical visits.
• Collaborates with outside facilities and clinical staff for certain patients, and provides general information about caring for patients to other caregivers including outside hospitals, PCP’s, VNA, and private help.
• Explore innovative ways to disseminate patient and community education materials to enhance continuity of care.

• Provide support and assistance with challenging patient/physician situations.
• Participate as requested in MGH and MGB quality initiatives.
• Participate in various hospital and system committees as assigned.

• Current licensure as an advanced practice in the Commonwealth of Massachusetts.
• Certification as Nurse Practitioner or Physician Assistant
