Summary: The Social Worker for the Mass General Hemophilia and Thrombosis Treatment Center (HTC) is part of a multidisciplinary team of clinicians providing care to children with bleeding disorders. This team includes physicians, nurse practitioners, rehabilitation care providers including physical therapists and other medical specialists. The program is supported by a Program Manager who oversees the financial aspects of the program and intervenes with families around insurance issues. Patients seen through this program include those with inherited or acquired bleeding and clotting disorders and are treated through a number of specialty clinics within the pediatric service. The Social Worker will provide clinical Social Work services through these outpatient programs. This is a part time (24 hour week) position reporting to the Social Work Department.
The Social Worker will develop ongoing relationships with patients and families and address issues of psychosocial concern for this population. This could include access to care, barriers to obtaining health care and compliance with care, impact of the social determinants of health, mental health challenges, child protective needs and adjustment to chronic health issues. The Social Worker would employ their skills including comprehensive biopsychosocial assessments, crisis intervention, ongoing supportive counseling, and collaboration with community programs to provide resources and support. The Social Worker will also develop relationships with current and new community programs which support these patients and families and engage with other regional social workers for continued education and collaboration.
Primary duties and responsibilities:
1. Evaluate the patient/family’s psychosocial situation including patient or staff concerns that affect the care of the patient
2. Provide crisis intervention, emotional support and short-term counseling to optimize the family’s coping and compliance with treatment.
3. Collaboration with the multidisciplinary treatment team to provide optimal care of the patient/family and enhance the patient/family experience.
4. Provide safety assessments as needed.
5. Participates in helping patients in the pediatric service transition care to adult providers as they “age out” of pediatric care.
6. Evaluate need and develop group programs for families to promote connections, emotional support, and education.
7. Collaborate with community programs to access resources and services for patients/families in need of assistance
8. Attend local, regional or national meetings and conferences which are relevant to this clinical specialty
9. Participate in quality assessment/improvement activities designed to evaluate the effectiveness of service delivery and quality of care.
10. Assist team in program planning activities.
11. May engage with bleeding disorder community to participate in advocacy events at the local or national level.
12. Maintains documentation of all interventions in the EPIC record.MSW Graduate
