Description Summary: Provides patient care to individuals with mental health diagnosis while working on a multidisciplinary team of MDs, APRNs, Nurses and Behavioral Health Clinicians. May be responsible for scheduling, rooming clients, and ensuring timely follow up in response to client needs.
Supervision Received
Works under the supervision of the Director of Practice Management reporting to the Clinical Practice Manager.
Essential Duties And Responsibilities
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
- Accurately obtain and record vital signs.
- Medication reconciliation.
- Administer mental health screenings including but not limited to SBIRT,PHQ9,CAGE,GAD and UDS ? Ensure all required elements are present in clients electronic health record (ie. W-10 discharge paperwork)
- Monitor Clozaril REMs and liaison with local pharmacy and labs to ensure patient compliance and to avoid disruption in pharmacologic therapy
- Complete prior authorizations for medications and DME.
- Assist with completing client required paperwork for disability, conservatorship, housing.
- Participates in multidisciplinary care team by effectively communicating with all stakeholders.
- Effective in client de-escalation.
- Performs other duties as required.
- All agency staff are required to attend all mandatory department/agency meetings and trainings*

- Ability to effectively utilize and navigate electronic health records.
- Effective communication skills.
- Effective prioritization and efficient time management.
- Ability to work in a shared work space with the treatment team.
Requirements
- Medical Assistant Certification or RMA
- Will consider Pharmacy Technician certification
- *one year experience preferred but not required
