The Manager, Clinical Pharmacy Services, under the direction of the Executive Director of Post-Acute Pharmacy, supports the objective of delivering safe, cost effective and optimal medication therapy. Responsible for direct oversight and supervision of the team members within their department. Responsible for overseeing and running, on a system level, clinical based pharmacy meetings such as but not limited to, Pharmacy and Therapeutics, Medication Safety Council, Pharmacy Clinical Committee, and other interdisciplinary Councils. Collaborate with pharmacy, facility, nursing and medical leadership on medication formulary development and adherence. The Manager, Clinical Pharmacy Services will attend, on a regular basis, local site P&T and QAPI meetings in order to help ensure consistency and observance to corporate clinical and quality decisions. Coordinate all clinical pharmacy services and programs to help obtain desirable patient outcomes through the use of safe, appropriate, and cost-effective medication therapies. Oversee clinical pharmacy operations across the pharmacy and its clients/facilities, promoting standardization and advancing clinical pharmacy practices.
Responsibilities
A day in the life of a Manager, Clinical Pharmacy Services at Hackensack Meridian Health includes:
1. Responsible for leading all aspects of the pharmacy’s clinical and quality programs; establishes and monitors key metrics of safety and clinical competency; provides coaching and training for new employees.
2. Works closely with the Executive Director and pharmacy leadership team; provides thought leadership and participates in strategy sessions for projects and priorities of the Post-Acute pharmacy department.
3. Leads/participates in Quality Assurance Committee discussions within facilities as well as pharmacy department; performs root cause analysis (RCAs) when a medication event occurs to prevent such occurrences in the future.
4. Oversees direct reports and maintains adequate staffing within the Clinical Pharmacy Department to meet departmental goals.
5. Trains new team members on the Clinical Pharmacy Department processes and procedures.
6. Performs analysis of current processes to help reduce the chances of errors while also identifying best practices and implementing such practice; maintains an effective program including development and implementation of strategies for increased medication variance reporting.
7. Serves as a clinical liaison to assist in the management of drug expense to budget by identifying opportunities for improvement and assisting in high-cost therapeutic alternative selection to achieve planned savings and/or cost avoidance; works with procurement and billing team to assist with all cost analysis for formulary considerations.
8. Collaborates with facility clinical leaders and administrators as well as network leadership; strives to develop a good rapport with the medical and nursing staff in addition to other multidisciplinary departments by actively seeking out opportunities to improve patient care and medication management.
9. Assists physicians and other healthcare providers in selecting the medication therapy that best meets the patient’s needs and contributes effectively to the overall therapy goals; applies specialized knowledge of the scientific and clinical use of medications,including medication action, dosing, adverse effects, and drug interactions.
10. Provides education to healthcare teams including pharmacists, nurses, physicians and students through in-services, presentations, lectures, clinical pearl memos and competencies. Educates healthcare team on new formulary medications, updated policies and procedures, medication safety initiatives and drug shortages.
11. Conducts regular huddles with Clinical Pharmacy Department team members.
12. Completes annual performance reviews and routine check-ins with direct reports.
13. Acts as a drug information specialist by responding to immediate medication needs/questions from nursing and/or physicians in a timely and accurate manner; develops alternative strategies for drug shortages; responsible for the dispensing, oversight, and management of high-risk medications.
14. Precepts pharmacy students and interns; provides practical experience and training to a pharmacy student including patient care rounds, topic discussions, seminar presentations, patient case presentations and journal clubs, etc.
15. Manages clinical pharmacy services and programs; oversees pharmacy Discharge Medication 16. Program by assisting in medication reconciliation and discharge education to select residents; partners with facilities to help develop and maintain successful Antibiotic Stewardship Programs (ASP) in compliance with state and federal regulations.
17. Demonstrates with ability to manage complex issues while maintaining a flexible, positive, cooperative demeanor.
18. Performs all other duties as assigned: Routine travel as required. Maintains and submits expenses for travel following expense reimbursement policies. Thinks and acts logically to meet unusual occurrences of the job in a professional manner. Maintains confidentiality of necessary information.
Qualifications
Education, Knowledge, Skills and Abilities Required:
1. PharmD or Bachelor’s of Science Degree in Pharmacy from an accredited School of Pharmacy.
2. Minimum of five (5) years of satisfactory, full-time experience as a licensed practicing Pharmacist, of which at least two (2) years were in an administrative, managerial, and supervisory capacity.
3. Successful leadership/management experience.
4. Client-facing consultation and/or business development experience.
5. Excellent communication skills, both written and verbal.
6. Excellent organizational and time management skills, with the ability to work in a face-paced environment, including the ability to to work under tight deadlines and autonomously.
7. Strong interpersonal skills with an ability to handle sensitive and confidential situations.
8. Good prioritization and multitasking skills.
Education, Knowledge, Skills and Abilities Preferred:
1. Knowledge of CMS and DOH regulations regarding Skilled Nursing Facilities.
2. Demonstrate experience with project management and oversight.
3. Clinical pharmacy program development.
4. Formulary management.
5. Data analytics and reporting trends.
6. Pharmacy automation technologies.
7. Experience in an academic healthcare setting.
Licenses and Certifications Required:
1. Registered Pharmacist License.
Licenses and Certifications Preferred:
1. Board Certified Geriatric Pharmacist (BCGP) from Board of Pharmacy Specialties
2. Certified Consultant Pharmacist (CCP)
3. Medication Safety Certification
4. Active New Jersey Immunization license
