The San Francisco VA is seeking dynamic leaders to fill our role as The Community Living Center (CLC) Medical Director.
The CLC Medical Director serves as a key member of the CLC leadership team providing direction on strategic goals and vision, making leadership decisions regarding the clinical well-being of the CLC residents and leading the interdisciplinary team of care providers. The medical director oversees and coordinates management of interprofessional staff providing care to veterans with complex chronic diseases and mental illness who reside at the CLC. This physician plays a leading role within the leadership team in helping to identify and address the effects of racism, sexism, xenophobia, homophobia, trans-phobia, ableism and other forms of discrimination on the health and well-being of veterans in the CLC and of CLC staff.
As a physician, the CLC Medical Director also provides primary care to CLC Veterans that includes assessment of patients, identification of medical problems, direction of medical management, participation in the development of individual plans of care during interprofessional care plan meetings, and determination of the need for other subspecialty services. This also includes end of life care.
The CLC Medical Director is expected to be a geriatric medicine expert with knowledge and practice experience in geriatric medicine and palliative care, dementia care, mental health illness management, rehabilitation, transitions of care, and healthcare systems navigation.
The CLC Medical Director leads and participates in quality improvement projects within the CLC and engages in SFVAMC committees, such as Infection Control, Transitions Referral and Coordination Team, Flu Campaign, P&T committee, National CLC calls, and other National efforts (e.g., CONCERT).
Serves as a consultant, collaborator, and contributor to the CLC interdisciplinary team and program.
The CLC Medical Director is a key member of the CLC leadership team responsible for CLC operations.
The CLC Medical Director is a supervisory position that supervises the Inpatient Subacute Rehabilitation Unit (ISRU) Medical Director, the CLC physicians and the CLC Recreation Therapy (RT) Supervisor.
