Title: Permanency Manager (KS-FCCM) Date: 7/29/2020 Revised: 5/26/2021 Department: Kansas Foster Care Agency: Cornerstones of Care Reports To: Director of Permanency Services FLSA Status: Exempt Pay Grade: 4 PURPOSE The Permanency Manager works under the direction of the Director of Permanency Services and is responsible for the supervision of Foster Care Case Management Specialists and ensures service compliance with the Kansas Foster Care Grant through the Department for Children & Families. SANCTUARY MODEL Cornerstones of Care is committed to the Sanctuary Model, a comprehensive trauma-informed method for creating change and helping individuals heal from traumatic experiences. It is a whole system approach built around the framework of SELF (Safety, Emotional Management, Loss, and Future). It is designed to facilitate the development of structures, processes, and behaviors on the part of staff, children and the community as a whole that can counteract the physical and psychological wounds suffered by the children, youth, and families in our care. The ultimate outcome of a trauma informed philosophy is the creation of safe, therapeutic living and working environments for our clients, families, agency members and ancillary providers which espouse the proactive principles of growth, change, and empowerment of the individual for the betterment of the community. Each employee across our family of agencies has the responsibility to demonstrate the tenets of the Sanctuary Model in their work with clients and in their interaction with other team members. Demonstrating these tenets is considered an essential responsibility for all staff in all positions across our agency. ESSENTIAL RESPONSIBILITIES * Supervise a team of Foster Case Management Specialists. * Provide back-up supervision to staff (as needed) in the absence of other Permanency Managers * Maintain responsibility for supervision of children assigned to direct reports * Accept cases from the Department for Children and Families (DCF) and assign to appropriate staff, within policy & procedure. * Be accessible by phone after hours, holidays and weekends, when assigned as the on call supervisor. * Lead team in achieving CFSR outcomes as identified in state grant. * Attend Case Plan and engagement meetings with staff as well as court hearings. * Approve and sign outgoing documents generated by staff including court reports, social histories, case plans, BIS documentation, adoption home studies, etc. * Serve as on-going liaison between Cornerstones of Care and funding sources, m aintaining adequate communication channels at all times to ensure that services remain flexible and creative in response to community needs. * Provide back-up case management services as necessary, consistent with contract guidelines and requirements. * Monitor outcomes and conduct utilization reviews. * Implement strategies to improve outcomes within the team as needed. * Advocate for the rights and needs of the children and their families and for the least restrictive community based services. JOB DESCRIPTION 2 * Be responsible for ensuring all policies, procedures, protocols, paperwork, data entry and practices are followed as outlined by Cornerstones of Care and DCF policy. * Provide supervision and evaluation of direct reports and other staff as needed and required. * Complete annual evaluations for employees on a timely basis per Cornerstones of Care policy and procedure. * Ensure, thorough supervision and monitoring each staff member maintains up-to-date records in the Cornerstones of Care and DCF management information systems for each child within established standards. * Monitor flow of paperwork in relationship to case file and any audits * Actively participate in the planning, development, and on-going growth of the program. * Attend and actively participate in scheduled program administrative meetings. * Facilitate team meetings and keep the team up-to-date on agency goals, objectives, policies, etc. * Be willing/able to travel as the job duties require. * Ensure compliance with all Cornerstones of Care and DCF standards and requirements, as set forth in policy, contracts/grants, and licensing documents. * Ensure all requirements are met for auditing purposes; including all service providers are meeting contractual agreements with Cornerstones of Care. * Be a part of a team decisions, provide input and suggestions related to assigned staff including hiring, termination, evaluation, and supervision. * Manage and/or participate in on-call process as dictated by Cornerstones of Care policy and procedure. * Provide team with professional development opportunities. * Professionally represent Cornerstones of Care both internally and externally to all stakeholders including communicating with supervisors and administrators across agencies at comparable levels. * Manage an allotted Service Budget for assigned team, and ensure expenditures are within approved budget amounts. * Provide monitoring and guidance to staff on effective ways to manage their individual case load budgets. * Respond to concerns from service providers, families, case team members and other professionals in a courteous and professional manner. * Communicate effectively and consistently with community service partners * Completes other duties as assigned by management and leadership teams REQUIRED KNOWLEDGE * Knowledge of computer skills and Microsoft Word, Outlook, Excel software processes. * Knowledge of Cornerstones of Care and DCF policies and procedures. * Knowledge of Cornerstones of Care contracts with external providers. * Knowledge of social work code of ethics. * Knowledge of child welfare practices and trauma-informed practices * Knowledge of community services * Knowledge of adoption through foster care and additional challenges related to this population REQUIRED SKILLS OR ABILITIES * Strong skills in direct service delivery, as well as overall management of services to children and their families aimed at achieving permanency and safety for children are necessary. * Strong financial management skills. * Creativity in service delivery is essential. * Excellent technology skills and the ability to be innovative in using technology to provide the most cost-and-time efficient social services to children and families. JOB DESCRIPTION 3 * Interpersonal Skills and teamwork: Work cooperatively and maintain professional relationships with co-workers and clients/customers, to promote a harmonious work and team environment. EXAMPLES OF DECISIONS MADE * Approves services with credentialed agencies. * Assessment of all direct reports. FORMAL POLICY-SETTING RESPONSIBILITIES * Provides input into policies and practices. MINIMUM REQUIREMENTS * Required: Bachelor’s degree in Social Work or other Human service related field and 2-5 years of work experience in child welfare case management services. Licensed by the Behavioral Sciences Regulatory Board to practice in Kansas (LBSW, LMSW, LSCSW, Professional Counselor, LMFT, and/or Alcohol and Drug Counselor). * Preferred: Master’s degree in Social Work or other Human service related field and 2-5 years of work experience in child welfare case management services. Licensed by the Behavioral Sciences Regulatory Board to practice in Kansas (LBSW, LMSW, LSCSW, Professional Counselor, LMFT, and/or Alcohol and Drug Counselor). In addition to the above requirements, incumbent must: * Complete Sanctuary training in a timely manner; * Have a valid driver’s license and proof of current vehicle insurance; * Follow all agency policies; * Be able to pass State Abuse, Neglect and criminal background checks; * Be able to carry out the essential functions of the position with, or without, accommodation. * Perform all job duties in a manner which demonstrates a commitment to Sanctuary values and practices. * Demonstrate awareness and understanding of cultural and ethnic diversity of clients, staff and other community partners.
