Title: Hospice and Palliative Care Registered Nurse

Primary Function/General Purpose of Position:
The Registered Nurse functions as the primary nurse for Noah’s Children families in
the home setting. He/she is a member of the Noah’s Children Interdisciplinary
team.
Employment Qualifications:
Graduate of an accredited school of nursing and possesses current licensure to practice as
a Registered Nurse in the Commonwealth of Virginia.
Is an advanced clinician who possesses a) broad based knowledge of clinical
psychosocial, and physiological theories relating to nursing diagnosis and treatment.
b) skills and ability to perform the rand of nursing procedures and techniques found in
general nursing practice, including the unusual and non-routine, and the ability to translate
these skills to the home setting; and c) demonstrated effective teaching skills. This level
of knowledge, skills, and ability may generally be acquired during 2 years current medical-
surgical staff nurse in acute care facility or during 12 to 18 months acute care plus 6 to 9
months current experience preferred.

Essential Job Functions
Initiates home cars assessment on assigned patients within 48 hours of admission to the program. Initiates the patients plan of care in accordance with nursing needs specified in the approved plan of care. Assesses patient/family needs on initial visit and completes the appropriate documentation including but not limited to the Initial Nursing Assessment, Pain Assessment and Medication Profile. Develops and updates the nursing care plans per program policy weekly. Re-evaluates patient /family with each visit at lease bi-weekly. Delivers skilled nursing services as indicated. Assesses patients/family needs for counseling and teaching of patient care skills; facilitates learning skills at home or on the unit if appropriate. Communicates relevant information to the Administrative Director and other team members as appropriate. Attends weekly Noah’s Children Interdisciplinary Team Meetings. Communicates with Medical Director and/or attending physician regarding patient’s condition, any relevant changes, and need or problems as appropriate. Assigns and supervises home health aides as needed and per program policy. Conducts in-home supervisory visits of home health aids bi-weekly and provides written instructions prior to the home health aides initial visit to the home. Demonstrates knowledge and practice of infection control and Safety Policy and Procedures. Maintains current standards of practice in delivering care. Coordinates acquisition of medical supplies and equipment as appropriate for patient/family. Participates in on-call rotation and responds appropriately to family/patient requests or needs. Provides documentation of all care provided in a timely manner and per Program Policy and Procedure. Coordinates support services (i.e. PT, OT, Dietary, etc.) under the direction of the Medical Director as appropriate for patient/family care. Demonstrates continuing participation in continuing education and in-service programs. Provides bereavement follow-up as requested. Maintains as submit accurate log of work- related activities. Demonstrates analytical ability required to gather and interpret patient data from multiple sources, apply theory, determine interventions needed and evaluate effectiveness of interventions for all patients seen on home visit and to assign and supervise the work of others including volunteers. Demonstrates the ability to work autonomously, utilizing sound judgment and a high level of nursing knowledge and expertise. Communicates effectively with patients/families from all social and economic background, and a high level of nursing’s knowledge and expertise, Demonstrates the skills and ability in using a variety of diagnostic and treatment related products, tools and manual and/or electronic devices (e.g., IV pumps, blood collection devices, mechanical lifts, wound care products, etc.) Demonstrates knowledge of all VA State Licensure Laws and “Medicare Hospice Benefit “conditions of participation. Demonstrates skills/intervention based on knowledge of death and dying issues. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrates knowledge of the principles of growth and development of the life span and posses the ability to asses data reflective of the patients status and interpret the appropriate information needed to identify each patients requirements relative to his or her age, specific need and to provide the care needed as described in the units departmental policies and procedures.
Other Job Functions:
Assists with the orientation and training of new staff as assigned
Participates in volunteer training as requested
Assists with office duties as requested
Assists with quality assurance activities as requested by the program director and is a
member of the QA&I Committee.
Covers the Hospice Office in the Directors absence.
Working Conditions:
Works in multiple community settings including offices, homes, schools, clinics, churches, and hospitals.
The individual performing this job may reasonably anticipate coming into contact with human blood, body fluids, and/or other potentially infectious materials. Individuals in this position are required to exercise universal precautions and use personal protective equipment and devices. Utilizes proper body mechanics and applies the policies concerning infection control.
Activities are non-sedentary. May assist in positioning, moving, and transporting patients. May be exposed to abusive, combative people and unsafe neighborhoods. Applies policies regarding electrical, chemical, radiation and back safety.
The working environment demands traveling throughout the hospital as well as in adjacent medical office buildings. Driving is required between the various hospital and facilities at any time of the day or night and within an established radius. Provided own transportation to/from patient’s homes and the office and carries valid automobile insurance. Mileage reimbursement will be according to BSRHS current policy.
May require provision of services in poor environmental conditions and/or high crime neighborhoods.

The following equipment will be utilized: computer (keyboard, screen ,printer) copier, telephone. Additional equipment will be required for specialty areas.
