Job Summary
- This position is responsible for accepting telehealth referrals, setting up patient care plans, performing video and telephonic patient visits, completing daily remote monitoring, troubleshooting program issues and coordinating care with other care providers.
- Responsible for patient follow-up and trouble-shooting of telehealth equipment and supplies.
Education and/or Experience
- Graduate from a CCNE or NLN accredited nursing program is strongly preferred. Graduate from an approved nursing program is acceptable.
- Minimum 2 years related healthcare experience and/or training preferred.
Licensure/Certification/Registration
- Licensed as a Licensed Practical Nurse (LPN) in the State of South Dakota and/or meeting the requirements of Multi-state licensure required.
- Licensed as a Licensed Practical Nurse (LPN) in the State of Minnesota required within 90 days of hire.
Work Schedule
- Varied Shifts; PRN, as needed
Pay
The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
- $22.50 – $31.25 per hour
