Ensure quality nursing care is provided within the guidelines of established facility policies and procedures, standards of care, and state and federal regulations.
Must be a currently licensed LPN in the State of Arizona.
Experience in Skilled Nursing Facility required.
At least one year of experience in a leadership role.
Knowledge of accepted techniques and practices for providing health care to the elderly/disabled.
Effective oral and written communication skills
Ability to establish and maintain rapport with physicians, ancillary service providers, residents, family members, and other staff.
Ability to supervise and coordinate nursing activities and staff.
Application of appropriate facility policies and procedures and long-term care rules and regulations.
Monitor the performance of nurses and nursing assistants in the provision of patient care.
Ensure documentation of resident status and change in condition/alert charting is consistent with facility policy.
Participate in the QAPI and Infection Control programs; follow through with recommendations.
Monitor effectiveness of treatment regimen, coordinate physician contact regarding changes in condition, consulting doctor recommendations, diagnostic testing results, incidents, and pharmacy/dietary recommendations.
Actively participates in staff meetings and ongoing educational in-services.
Monitor adherence to facility personnel policies and procedures.
Work with nursing staff to resolve patient/family concerns using a proactive approach.
Assist with training of new staff, skill competencies, and ongoing education/in-servicing.
Assist with reviewing referrals, entering admit orders, admissions, discharges, and transfers when necessary.
Maintains records of medication room refrigerator temperature.
Keeps med carts and room free from open, undated vials and outdated meds. Accountable for narcotic count and records.
Provides leadership/guidance for the facility during shift
Assist with patient care as necessary.
Requires standing and walking frequently throughout the working day.
Job involves reaching, bending, stooping, bending, heavy lifting, and carrying equipment and supplies.
Involves moderate exposure to unusual elements such as extreme temperatures, dirt, dust, fumes, smoke, unpleasant odors, and/or loud noises.
