Overview of the Role
The Alignment Care Anywhere program is a Physician led, Advanced Practice Clinician (APC) driven, home-based program for chronic disease management. The program is designed to provide care and support to our Alignment Healthcare patients by providing an additional level of medical and social engagement in the comfort of their own home. Engaging with members in their home environment allows for detection of potential health risks and care management opportunities with the goal of preventing health complications and unmanaged disease progression. Effective engagement with our members leads to overall improved healthcare outcomes and allows them to remain at home, reducing unnecessary hospitalizations and emergency room visits. At Alignment Healthcare we focus on the senior and provide care with a serving heart.
Responsibilities
Perform in-home comprehensive assessments on qualifying Alignment Healthcare members including timely completion and submission of all required documentation.
Review medications and order pharmacologic interventions as needed.
Identify disease specific or preventive diagnoses to be used in care management and active medical management and document in the medical record in a manner complaint with CMS regulations.
Perform, document, and communicate results of Point of Care (POC) Testing.
Communicate findings in your assessment that will be used to generate a post-visit summary note to the PCP and identify potential gaps in care.
Make general recommendations to members intended to improve members’ knowledge of their chronic condition(s), such as information concerning recommended testing and medication compliance.
Recognize urgent situations requiring escalation and take appropriate action as determined by reasonable professional judgment and ethical professional practice standards.
At the direction of the Regional Medical Officer, review outcomes and utilization data, including hospitalizations, and use of specialists to establish best practices and policy improvements.
Participate in weekly interdisciplinary team meetings to discuss and develop the most appropriate care plans possible based on the needs of our members/patients.

Required Skills & Experience
Preferred one (1) year prior clinical or home care experience (NEW GRADS WELCOME)
Previous EMR experience preferred
Master’s degree from an accredited NP Program or PA program
Active Nevada state Nurse Practitioner or Physician Assistant license
Active RN License (Must, upon hire) NP only
Furnishing number (Must, upon hire) NP only
Active Nurse Practitioner Board Certification/Physician Assistant Certification
NPI Number (Must, upon start)
DEA Number (Must, within 6 months of start)
Valid BLS (Basic Life Support) – required upon start
ACLS (Advanced Cardiovascular Life Support) – preferred
Valid Nevada driver license and current automobile insurance
Ability to work independently
Experience in care of older adult (geriatric) patients preferred
Excellent administrative, organizational, and communication skills required
Knowledge of Microsoft Office applications
Knowledge of CMS and NCQA HEDIS Guidelines
Knowledge of Evidence-based Clinical Practice Standards: American Diabetes Association (ADA), American College of Cardiology (ACC), American Heart Association (AHA)
Familiarity with ICD-9, ICD-10 and CPT-4 coding
Comply with all HIPAA regulations and maintain security of protected health information (PHI)
PAY RANGE: $118,000 – $177,000 annually
