Job Responsibilities
- In coordination with the healthcare team, assesses, plans, directs, evaluates, and modifies each resident’s care plan to meet with his/her physical, psychosocial, and cultural needs
- Manage emergency situations in a prompt, precise, and professional manner
- Maintains accurate, detailed reports and records
- Completes medication administration and treatment needs for all assigned residents
Certifications & Key Qualifications
- Must possess an LPN license in the state of PA upon hire
- Have experience in the long term care setting or the drive to work in a fast paced environment
- Treat each resident with care, compassion, and respect
- Are patient team players with the ability to adapt to easily to change
- Communicate effectively with the residents you care for as well as the nursing team
- Embrace our person-centered care approach to meet each resident’s needs and preferences
