Help address the social determinants of health and help hospital inpatients and families maximize their health and welfare by educating and connecting them to community resources and public benefits. Our Community Resource Specialists collaborate with social worker colleagues to assess patient/family resource needs, educate about options, facilitate appropriate referrals and empower patients and families to advocate for themselves. Examples include educating about various types of medical employment leaves and disability programs (MA PFML, SSI/SSDI, EAEDC) eligibility and how to apply, helping establish transportation (PT-1 and The RIDE medical necessity applications) and teaching how to use these systems, serving as a liaison with the adult shelter system, and helping identify, access and trouble-shoot a wide range of other social service systems and resources (not a complete list). Great opportunity to learn about community resources and advocacy, and gain exposure to social work and the health care field.

Salary & Benefits:

Hours: M-F 8:30am – 5:00pm (40 hours).
