Overview
Western Reserve Medical Group is a medical house call practice which serves our senior & IDD population in their home environment. As a member of our Western Reserve Medical Group (WRMG) team, you’ll be responsible for performing Chronic Care Management (CCM) services for our chronically ill patients in accordance with CMS guidelines.
We are looking for Certified/Registered Medical Assistants, licensed practical nurses, or other similarly qualified candidates to work with our Medicare population. Familiarity with Chronic Care Management and care planning is strongly preferred. The candidate must be efficient while providing compassionate, patient-centered care. Customer service skills are a must while building rapport through meaningful, relevant patient interaction over the phone. s a daily user of technology, the candidate needs to be comfortable accessing and troubleshooting changing technologies as necessary.
The candidate will use their health care experience, talents, and problem-solving skills to work independently and manage a monthly allocation of patients ensuring that all assigned patients are cared for. They will utilize multiple platforms to ensure service is consistent and compliant while developing or revising individualized comprehensive care plans. With a goal of speaking with 90% or more of the allocated patients, the candidate will make multiple attempts to contact patients to provide the health care team with the most up-to-date information. Although the candidate will be working independently, they will be a team member alongside other like-minded, compassionate health care workers who support each other and are dedicated to excellent patient-centered care.
The Chronic Care Coordinator role requires a candidate that is comfortable providing non-face-to-face (i.e., telephonic) patient care as an extension of the clinical team. This role includes providing monthly assessments and medication reviews, coordination with family members and caregivers and follow-up with other office support staff. After an appropriate paid training, this position will be 100% remote unless additional training is needed.
