Nursing Coordinator Transfer Center Per Diem Position Summary
(General statement reflecting the overall purpose of the position)
The Cord, Nursing coordinates a patient-centered admission process for patients transferring into the Abrazo Community Health Network/Carondelet System ensuring placement in an appropriate level of care bed and financial screening per EMTALA regulations. They employ critical thinking skills to expedite admissions into ACHN/Carondelet while demonstrating ACHN/Carondelet’s Mission, Vision, Values and Service Standards in all interactions with referring facilities, physicians, employees and guests.
SPECIAL SKILLS:
Organizational and leadership skills
Ability to decipher specific messages with background noise.
Effective communication skills – phone etiquette, written and verbal – with all levels of staff, physicians, patients and families.
Experienced at multi -tasking.
Ability to present self in positive and professional manner, exhibiting confidence, tact, and enthusiasm.
Must be sensitive to diverse cultural and bilingual issues
Adept at interpreting and analyzing qualitative and quantitative data
Ability to quickly triage, critically think in high pressure situations and prioritize
THE NURSING COORDINATOR TRANSFER CENTER PER DIEM CANDIDATE WILL POSSESS THE FOLLOWING EDUCATION, LICENSE/CERTIFICATIONS, AND EXPERIENCE.
EDUCATION
Required: Graduation from accredited nursing program
Preferred: BSN preferred
WORK EXPERIENCE
Required: Minimum of 3 years continuous clinical experience in a critical/acute care setting with adult neuro/trauma/cardiovascular/critical care or emergency room nursing preferred. Ability to manage initial assessment data and prioritize patient care with appropriate decision making.
Preferred: 8 years of critical care, emergency room, or house supervisor experience
CERTIFICATIONS/LICENSURE/REGISTRATION
Required: Registered Nurse with current state licensure. Current BLS certification required;
Required: EMTALA Training through Tenet HealthStream
Preferred: ACLS training preferred.
